Patient Aid program
Fight 4 Cure Inc’s Patient Aid grants are now awarded through an open application process and funding is provided to adult cancer patients with a verified cancer diagnosis and treatment plan.
Guidelines
Patient Aid grants consist of a one-time $100 grant.
Due to the increase in requests for our services, Patient Aid applications for each calendar year will be accepted on a quarterly basis. Deadlines for submissions are as follows:
Quarter 1: February 15th - Until Fully Subscribed
Quarter 2: May 15th - Until Fully Subscribed
Quarter 3: August 15th - Until Fully Subscribed
Quarter 4: November 15th - TBD based on funding availability
Applications received via U.S. Postal Services (USPS) after the program is FULLY SUBSCRIBED will no longer be accepted unless prior arrangements have been made for reasonable accommodations.
Eligibility Checklist
Adult cancer patients applying must meet the following criteria:
Must be U.S. Citizen or permanent resident and reside in the U.S. (Note: a Social Security Number (SSN) or Alien ID Number is required on all applications).
Must have a confirmed cancer diagnosis. (Note: All diagnosis and treatment plan will be verified through current healthcare team).
Must be in active treatment, scheduled to begin treatment or in follow up care (Note: Follow up care timeframe is 3 years or less and consists of blood work, scans, etc. Hormone therapy alone and hospice care are not considered active treatment nor follow up care).
Must not have received a Patient Aid grant in the past.
Approval Process
The Patient Aid Manager and the President will review the application and contact the healthcare team to obtain a fax number. Afterwards, the patient’s signed HIPAA authorization will be faxed to the number provided to request a letter of support verifying diagnosis and current treatment plan.
The President may set an established deadline in which the healthcare team is expected to respond and/or provide the letter of support. (Note: The Patient Aid Manager and the President reserve the right to deny financial assistance if the healthcare team has not responded to the request for a letter of support within 30 days).
Applicants will be notified of any decision via official USPS mail. (Note: all decisions are not final and can be reconsidered within 45 days provided the appropriate documentation is received from the healthcare team. If an appeal is not received within 45 days, the patient must reapply during the appropriate funding cycle outlined for each quarter).
If an applicant is approved, grant checks will be mailed directly to the address listed on the Patient Aid Program application. Please allow up to 30 days for delivery. (Note: if a grant is returned to the organization as undeliverable, the patient will be contacted via email to reverify their mailing address. If the patient does not respond within 2 weeks of being contacted, the grant will be voided and patient will need to reapply).
As a nonprofit organization, funding for this program relies on the generosity of donor support. Hence, the ability of Fight 4 Cure Inc to distribute grants of this nature is dependent on the availability of funds which can result in the program being modified or discontinued at any time.
